We are looking for compassionate support workers to provide non-judgmental support to young people who may have issues around past trauma, drug or alcohol misuse, mental health diagnosis and poor motivation.
Phase 1 is designed to support young adults (16+) who may be care leavers and have reached adulthood without having developed the skills needed to live independently.
As a support worker, you will follow a person-centred support plan to assist the individuals with all aspects of daily living to promote and achieve independence. Day to Day, you will assist with Meal Preparation, employment or further education applications, budgeting finances, personal Hygiene, booking/attending appointments, Support with medication, welfare checks.
Phase 2 allows us to continue our support as our young people successfully move on into the community. We may provide welfare checks, telephone support or be a friendly ear.
In Abergele, our support workers will also provide support to the wider community in their own homes. Support workers must be kind, respectful and compassionate as they meet personal care needs, support with medication and provide social engagement for vulnerable people.
